动态动脉乳酸水平与手足口患儿预后的相关性研究

摘要：

目的：探讨监测动脉乳酸水平的动态变化对手足口病患儿预后评价的临床价值。方法：按照APACHE II评分将651例手足口病患儿分为非危重组、危重组及极危重组，分析各组1h、12h、24h及48h的动脉乳酸动态变化,比较入院1h内各组间动脉乳酸水平的差异及不同预后组动脉乳酸水平的差别。结果：极危重组动脉乳酸水平高于另2组并呈持续升高态势；而另外2组动脉乳酸水平较低，且随着治疗的进展逐渐下降。APACHE II评分越高，动脉乳酸浓度越高（r=0.67，P＜0.01）；动脉乳酸浓度越高，预后越差。结论：动态监测动脉乳酸水平是判断手足口病预后的一个良好指标，动脉乳酸越高，预后差。

Objective：To study the clinical value of blood lactic acid level of patients in HFMD in chidren.Methods According to APACHE II system,651 patients were divided into three grou ps:non-critical group,critical group and exetremely critical group. Analysis of each group 1h, 12h, 24h and 48h of the dynamic changes of blood lactate .Blood lactic acid level were compared among these groups and among different prognostic.Results Exetremely critical group’s blood lactate levels higher than the other two groups and showed increasing trend continued; while the other two groups a lower blood lactate levels, and decreased gradually with the progress of the treatment.The higher APACHE II was, the higher level of blood lactic acid was（r=0.67，P＜0.01）.The prognosis of the patiens had a correlation with the level of blood lactic acid.Conclusion The blood lactic acid level is a useful index for the prognosis evaluation in HFMD complications of encephalitis patients. The higher the blood lactic acid level the patients have , the worse prognosis they have ,especially when the blood lactic acid value has been in high level for a long time.
