新生儿高胆红素血症对心肌酶影响的探讨

摘要:

摘要 目的 探讨新生儿高胆红素血症对心肌酶活性的影响。 方法 选择高胆红素血症的住院新生儿60例作为治疗组,40例同期出生的正常新生儿作为对照组,分别进行胆红素、心肌酶四项AST CK CK-MB LDH、肌钙蛋白T、及心电图检测。结果 新生儿胆红素越高心肌酶活性越高,治疗前和治疗后心肌酶活性比较有显著性差异P<0.01,随着胆红素的降低,血清心肌酶活性明显降低,与正常对照组之间差异无显著性P>0.05。结论 新生儿高胆红素血症能引起心肌酶活性的升高,但不一定造成患儿心肌细胞不可逆损伤,儿科医生在判断黄胆对心肌有无损伤时除进行心电图检查外,还要将心肌酶和肌钙蛋白T列为常规检测项目,避免新生黄胆患儿因心肌损伤而死亡。

[Abstract] Objectine :To discuss the myocardial enzymes activity effect of the neonatal hyerbilirubinemia . Methods:60 cases with neonatal hyperbilirubinemia as a treatment group ,40 cases of the same period replacement ot the normal newborn as a control group .To monitor the two groups before and after enzyme AST CK CK-MB LDH CTNT ECG values.and to compare and analyze .Results :The higher the concentration of bilirubin enzyme activity was also high.and with the decline in total bilirubin level.the myocardial creatine phosphate and creatine kimase-MB was also gradually declining .before and after treatment enzymogram value of the treatment group were compared and there were significant difference (P<0.01). bilirubin level decline, myocardial enzymes also decline,between of normal group no difference(P>0.05).Conclusion: the neonatal hyerbilirubinemia cause rise of myocardial enzymes ,but myocardial cell uncertain irreversible damage. Pediatricians judge harm of neonatal hyperbilirubinemia to cardiac muscle except ECG neonatal jaundice in children should be as routine examination enzymogram projects to avoid children with neonatal jaundice and death due to myocardial damage.
