EV71感染重症手足口病患儿循环障碍特点及治疗

摘要：

目的：观察分析肠道病毒71型（EV71）感染所致重症手足口病患儿临床表现及循环障碍发生的特点及治疗方法。方法：回顾性分析186例重症手足口病患儿为研究对象，其中EV71感染者53例，柯萨奇病毒A组16型（COA16）感染者97例，其他病毒感染者36例，根据感染病毒不同分为EV71组、CA16组、其他病毒组，观察三组患儿循环障碍发生等临床特点、治疗方法及预后。结果：EV 71组患儿循环障碍、神经系统受累、肺出血临床发生率高于CA16组及其他病毒组，EV71组中心动过速、血压增高、末端循环障碍为EV71组最为常见的循环障碍临床表现，且发生率高于CA16组及其他病毒组，EV71组住院时间长于CA16组及其他病毒组，差异均有统计学意义（P<0.05）。结论：EV71感染重症手足口病患儿神经系统受损、肺出血、循环障碍发生率高于其他病毒感染，血压不稳、心率加快是其主要循环障碍临床表现，有效的预防措施能够有效的防止EV71感染患儿短时间内发展为危重病例。

Objective: To observe and analyze the enterovirus 71 (EV71) infection features and treatment of children with severe hand, foot and mouth disease clinical manifestations and circulatory disturbance occurred.Methods: A retrospective analysis of 186 cases of children with severe hand, foot and mouth disease for the study, of which 53 cases of EV71 infection, Coxsackie virus A group of 16 type (COA16) infection in 97 cases, 36 cases of other viral infections, according to the virus EV71 were divided into groups, CA16 group, the other group of viruses, clinical characteristics of the three groups of children and other circulation disorder, treatment and prognosis. Results: EV 71 group of children with circulatory disorders, neurological involvement, the high incidence of pulmonary hemorrhage in CA16 group and other groups, EV71 virus group tachycardia, blood pressure, circulation disorders are the most common end of the cycle of EV71 group of clinical manifestations of disorder and higher than the CA16 group and other group had the virus, EV71 group in hospital longer than CA16 group and other viral groups, the differences were statistically significant (P<0.05). Conclusions: EV71 infection in children with severe hand, foot and mouth disease nervous system damage, pulmonary hemorrhage, circulatory disorders was higher than other viral infections, unstable blood pressure, psychological accelerate its main clinical manifestations of circulatory disorders, effective preventive measures that can effectively prevent EV71 infection the development of critical illness in children in a short time.
