rhG-CSF联合基础治疗对急性放射性口腔黏膜炎患者KPS、NRS评分及炎症因子的影响

摘要:

目的:观察重组人粒细胞集落刺激因子(recombinant human granulocyte colony-stimulating factor,rhG-CSF)联合基础治疗对急性放射性口腔黏膜炎患者卡氏评分(KPS)、疼痛数字评价量表(numerical rating scale,NRS)评分及炎症因子的影响。方法:选择2014年3月至2016年3月我科收诊的80例急性放射性口腔黏膜炎轻度疼痛患者作为研究对象,按照随机数字表的方法分为试验组和对照组,对照组患者给予生理盐水、地塞米松、庆大霉素和利多卡因进行基础治疗,试验组患者在对照组的基础上给予rhG-CSF进行治疗,比较两种治疗方式对两组患者的KPS、NRS评分和炎症因子TNF-α、IL-6、IL-8的影响。结果:放疗后两组患者KPS均明显下降,且对照组患者KPS明显低于试验组患者[(36.96±5.30)分vs(47.81±4.01)分,P<0.01];放疗后两组患者NRS评分均明显上升,且对照组患者NRS评分明显高于试验组患者[(5.70±1.08)分vs(3.71±0.69)分,P<0.01];放疗后两组患者TNF-α、IL-6及IL-8水平均明显升高,且对照组患者TNF-α、IL-6及IL-8水平明显高于试验组[(29.42±2.71) ng·L-1 vs(21.35±2.01) ng·L-1;(30.63±2.88) ng·L-1 vs(21.90±2.05) ng·L-1;(4.79±0.52) ng·L-1 vs(3.62±0.40) ng·L-1P<0.01]。结论:rhG-CSF能够降低急性放射性黏膜炎患者炎症反应,缓解患者口腔疼痛感,有效改善患者体力状况。

Objective: To observe the effect of recombinant human granulocyte colony-stimulating factor (rhG-CSF) combined with conventional treatment on Karnofsky performance status(KPS), numerical rating scale (NRS) score and inflammatory factors of patients with acute radioactive oral mucositis.Methods: From Mar.2014 to Mar.2016, 80 patients with acute radioactive oral mucositis with mild pain were randomly divided into the experimental group and the control group. Patients in the control group were treated with saline, dexamethasone, gentamicin and lidocaine for conventional treatment, while the experimentalgroup was treated with rhG-CSF on the basis of conventional treatment. KPS, NRS score and inflammatory factors were compared between two groups. Results: After radiotherapy, the KPS scores of two groups were significantly decreased, and the KPS scores of the control group(36.96±5.30) was significantly lower than those of the experimental group(47.81±4.01), P<0.01; After radiotherapy, the NRS scores in both groups were significantly increased, and the NRS scores of the control group(5.70±1.08) was significantly higher than those of the experimental group(3.71±0.69), P<0.01; After radiotherapy, the levels of TNF-α, IL-6 and IL-8 of two groups were significantly increased, and the levels of TNF-α, IL-6 and IL-8 of the control group[(29.42±2.71), (30.63±2.88), (4.79±0.52) ng·L-1] were significantly higher than those of the experimental group[(21.35±2.01),(21.90±2.05),(3.62±0.40) ng·L-1],P<0.01.Conclusion: rhG-CSF can reduce the inflammatory reaction of patients with acute radioactive oralmucositis, relieveoral pain of patients, and effectively improve the physical condition of patients.
