肠内营养对高龄糖尿病合并心力衰竭患者的疗效观察

摘要:

目的:观察肠内营养对高龄糖尿病合并充血性心力衰竭(CHF)患者的临床疗效。方法:选择60例80岁以上糖尿病合并慢性CHF且存在营养风险(NRS-2002评分≥3分)的患者,随机分为对照组(予常规治疗加饮食健康教育)与营养干预组(在常规治疗及饮食健康教育基础上加肠内营养,持续3个月)各30例,观察两组治疗前后左室射血分数(LVEF)、心输出量(CO)、血清白蛋白(Alb)、血红蛋白(Hb)、前白蛋白(PAB)、6 min步行距离(6 MWD)等指标。结果:营养干预组治疗后LVEF、CO、Alb、Hb、PAB、6 MWD指标较治疗前有明显改善(P<0.05),且均优于对照组治疗后(P<0.05)。结论:肠内营养能改善存在营养风险的高龄糖尿病合并慢性CHF患者的心功能,提高生活质量。

Objective: To observe the effect of enteral nutrition on the elderly diabetic patients with congestive heart failure. Methods: Sixty elderly diabetic patients with heart failure and malnutrition risks were collected and all the patients were more than 80 years old. They were randomly divided into control group(normal therapy and dietary education for three months) and treatment group(enteral nutrition treatment for three months in addition to normal therapy and dietary education). Left ventricular ejection fraction(LVEF), cardiac output(CO), serum albumin(Alb), hemoglobin(Hb), prealbum (PAB) and 6 min walk distance(6 MWD) were compared before and after the treatment in each group. Results: After enteral nutrition treatment, the LEVF, CO, Alb, Hb, PAB and 6 MWD were significantly improved in the treatment group(P<0.05). Furthermore, the above indexes in treatment group were much better than those in the control group(P<0.05). Conclusion: Enteral nutrition could improve the cardiac function and the quality of life in elderly diabetic patients with congestive heart failure.
