慢性心力衰竭患者肌酐水平与抑郁相关性研究

摘要:

目的:研究慢性心力衰竭(CHF)患者血清中肌酐水平与抑郁的相关性。方法:选取2016年8月至2017年9月于陕西省人民医院心内一科住院治疗的336例CHF患者。根据肌酐水平分为肌酐正常组(n=168)和肌酐异常组(n=168),收集两组患者的一般资料及临床检验结果,采用汉密尔顿抑郁量表评估抑郁程度,并对血清肌酐水平与抑郁评分相关性进行简单线性相关分析,对心衰患者抑郁相关因素进行Logistic回归分析。结果:肌酐异常组的抑郁评分显著大于肌酐正常组[(14.81±10.94)分vs(11.57±8.84),P<0.001],简单线性相关分析显示肌酐水平与抑郁评分呈正相关(r=0.190,P<0.001);Logistic回归分析显示,在校正年龄、高密度脂蛋白胆固醇、B型脑钠肽、红细胞计数、糖化血红蛋白因素后,肌酐是CHF并发抑郁的显著相关因素(OR=1.024,P=0.001)。结论:在CHF患者中肌酐与抑郁有关。

Objective: To investigate the association between creatinine and depression in patients with chronic heart failure (CHF). Methods: A total of 336 hospitalized patients with CHF in the First Department ofCardiology of the People's Hospital of Shaanxi Provincewere enrolled from August 2016 to September 2017.They were divided into two groups:normal creatinine group (n=168) and abnormal creatinine group (n=168).The general clinical data and laboratory results of two groups were collected, the Hamilton Depression Scale was applied to evaluate the degree of depression,the data above were analyzed using Pearson linear correlation analysis and Logistic regression analysis. Results: In abnormal creatinine group, depression score was significantly higher than that in the normal creatinine group[(14.81±10.94) scoresvs (11.57±8.84) scores, P<0.001], Pearson linear correlation analysis showed that creatinine level was positively correlated with depression score (r=0.190, P<0.001). Logistic regression analysis showed that after adjustment of age, high density lipoprotein,brain natriuretic peptide, red blood cell count, glycosylated hemoglobin, creatinine level is significantly associated with depression score in CHF patients (OR=1.024, P=0.001).Conclusion: Creatinine is associated with depression in patients with CHF.
