目的：了解TPO抗体升高类风湿关节炎患者的血清炎症细胞因子[白介素-2（IL-2）、白介素-6（IL-6）、肿瘤坏死因子-α（TNF-α）]的水平，分析患者的临床特点。方法：将符合纳入标准的80例患者分为TPO抗体正常类风湿关节炎组（TPO抗体正常组）和TPO抗体升高类风湿关节炎组（TPO抗体升高组），每组40例。比较两组性别、年龄、病程、HAQ评分以及DAS28评分等资料，运用ELISA方法检测两组血清炎症细胞因子水平。结果：两组病程、HAQ评分和DAS28评分差异均无统计学意义（P>0.05）。TPO抗体正常组年龄为（59.0±9.8）岁，大于TPO抗体升高组的（53.6±9.7）岁（P<0.05）。TPO抗体升高组TNF-α水平为583.3 pg·ml^-1，明显高于TPO抗体正常组的284.9pg·ml^-1（P<0.05）。两组IL-2、IL-6水平差异均无统计学意义（P>0.05）。结论：TPO抗体升高类风湿关节炎患者的年龄相对小，其血清TNF-α水平较高，甲状腺功能也异常。
