外周血miR-323-3p在异位妊娠早期诊断中的作用

摘要:

目的:探讨外周血miR-323-3p在异位妊娠早期诊断中的作用。方法:选取我院收治的45例异位妊娠患者与同期45例正常宫内妊娠者进行对比,分别用酶联免疫法、二代测序技术和实时荧光PCR技术检测血清中β-人绒毛膜促性腺激素(HCG)和血浆中miR-323-3p的含量,利用受试者工作曲线(ROC曲线)分析外周血miR-323-3p、β-HCG及二者联合检测对异位妊娠的早期诊断价值,包括曲线下面积、灵敏度和特异度等。结果:异位妊娠组与正常宫内妊娠组在β-HCG 48 h增长率和miR-323-3p相对表达水平方面差异显著(P<0.05),且异位妊娠组有较低的β-HCG 48 h增长率和较高的miR-323-3p相对表达水平;外周血miR-323-3p与β-HCG的检测水平无明显相关性(P>0.05);外周血miR-323-3p对早期异位妊娠的诊断价值(敏感度86.7%、特异度72.3%)明显高于β-HCG 48 h增长率的诊断价值(敏感度68.6%、特异度70.2%),且当二者联合检测时,诊断价值最高(敏感度91.4%、特异度88.5%)。结论:外周血miR-323-3p对异位妊娠具有较好的早期诊断价值,临床上可以考虑miR-323-3p和β-HCG的联合检测。

Objective:To investigate the diagnostic value of miR-323-3p in early ectopic pregnancy. Methods:A total of 45 cases of ectopic pregnancy patients and 45 normal intrauterine pregnancy women were selected as ectopic pregnancy group and normal pregnancy group, respectively. The levels of serum β-HCG were detected by enzyme linked immunosorbent assay (ELISA), miR-323-3p levels were detected by next-generation sequencing (NGS) and RT-qPCR assay. The diagnostic value of serum β-HCG and miR-323-3p for ectopic pregnancy was determined by ROC curve. Results:The differences were significant in β-HCG 48 h growth rate and miR-323-3p relative expression between the ectopic pregnancy group and the normal pregnancy group (P<0.05). The 48 h growth rate of β-HCG was lower and the relative expression level of miR-323-3p was higher in ectopic pregnancy group compared to the normal pregnancy group. There was no significant correlation between the level of miR-323-3p and β-HCG in peripheral blood (P>0.05). The results of ROC curve analysis showed that the diagnostic value of miR-323-3p in peripheral blood for early ectopic pregnancy was significantly higher than that of β-HCG 48 h growth rate, and the diagnostic value was the highest when the two were combined. Conclusion:Peripheral blood miR-3233p has good early diagnostic value in ectopic pregnancy, and the combined detection of miR-323-3p and β-human chorionic gonadotropin can be considered clinically.
