目的：提高艾滋病相关隐球菌性脑膜炎的救治水平。方法：回顾性分析本院2015年12月至2018年5月收治的4例重症艾滋病合并隐球菌性脑膜炎的临床资料，应用正规抗隐球菌治疗结合腰大池腹腔分流手术，追踪其疗效。结果：4例患者均为男性，年龄20~30岁，病程10 d~1个月，4例患者均伴有发热、剧烈头痛，3例伴有视力障碍，3例伴有抽搐及意识障碍；经抗隐球菌及腰大池腹腔分流手术治疗，4例患者颅高压症状缓解，2例视力恢复如前，1例好转，无其他并发症出现。结论：腰大池腹腔分流术能安全有效持久地控制颅高压，减少并发症及后遗症的出现。
