全肠外营养与肠内加肠外营养在胃癌患者中应用效果的回顾性研究

摘要:

目的:比较不同营养支持方式对胃癌手术患者的应用效果。方法:回顾性研究2015年1月至2017年12月于本院胃肠外科行手术治疗的术后分别行全肠外营养(TPN)和肠内加肠外营养(EN加PN)的86例胃癌患者的相关临床数据。对两组患者在术后7 d外周静脉血指标,术后是否输注白蛋白,术后是否输血,术后住院时间,住院总费用,术后进流质饮食时间,术后进半流质饮食时间,以及并发症等方面进行分析及比较。结果:两组患者在术后7 d外周血TBil、DBil、ALT、AST、γ-GT、Na+、Ca2+、TP水平,术后是否输血,进食半流质饮食时间方面差异均无统计学意义(P>0.05);在Alb、K+,术后是否输注白蛋白,术后住院时间,住院总费用,术后进流质饮食时间以及并发症发生率方面差异均有统计学意义。结论:肠内加肠外营养支持方法在术后营养支持方面更有优点,应作为胃癌术后患者的首选营养支持法。

Objective: To compare the effect of different nutritional support methods on patients with gastric cancer. Methods: 86 patients with gastric cancer who underwent gastrointestinal surgery from January 2015 to December 2017 in the Union Hospital of Tongji Medical College, Huazhong University of Science and Technology, were divided into two groups:total parenteral nutrition (TPN) group and enteral + parenteral nutrition (EN+ PN) group. A comparison and analysis was made between the two groups in terms of blood TBil, DBil, LT, AST, γ-GT, K+, Na+, Ca2+, TP, Alb level on the 7th days after operation, whether albumin was infused after operation, whether blood transfusion after operation, postoperative hospital stay, total hospitalization costs, time of intake of fluid diet, time of intake of half fluid diet, and complications. Results: There were no statistical difference (P>0.05) between the two groups in the following aspects:the blood TBil, DBil, ALT, AST, γ-GT, Na+, Ca2+, TP level on the 7th days after operation, whether blood transfusion after operation, time of intake of half fluid diet. There were statistical difference (P<0.05) between the two groups in the following aspects:the blood Alb level,K+ level on the 7th days after operation, whether albumin was infused after operation postoperative hospital stay, total hospitalization costs, time of intake of fluid diet and complications. Combined enteral and parenteral nutrition was better than total parenteral nutrition in those respects. Conclusion: Both kinds of nutritional support methods are suitable for patients with gastric cancer after operation, but the combined enteral and parenteral nutrition support method is better than total parenteral nutrition support.
