颏下岛状皮瓣修复治疗口腔颌面部肿瘤临床疗效分析

摘要:

目的:分析颏下岛状皮瓣在治疗口腔颌面部肿瘤中的应用价值,观察临床疗效。方法:择取2013年6月至2016年6月我院收治的36例口腔颌面部肿瘤患者,依据患者是否存在放疗禁忌证和是否自愿接受放疗,将所选患者分成对照组(18例)和研究组(18例),对照组行口腔颌面部肿瘤的扩大切除术+颏下岛状皮瓣修复治疗术+淋巴结清扫术,研究组在对照组手术治疗的基础上再实施肿瘤放射治疗,对比分析两组患者的皮瓣成活情况、术后复发率、生存率、术后并发症及患者满意度。结果:研究组皮瓣成活率(94.44%)与对照组皮瓣成活率(88.89%)比较无统计学意义(P > 0.05),同时研究组术后复发率低于对照组;术后1年、2年、3年生存率比较无明显差异,研究组总满意度高于对照组,分析存在统计学差异(P<0.05)。术后并发症发生概率比较无显著差异(P > 0.05)。结论:口腔颌面部肿瘤患者行以颏下岛状皮瓣治疗,同时进行术后放疗,可取得良好效果,不会对皮瓣存活产生严重影响,既可保证皮瓣成活率,又可使临床肿瘤复发率得以控制,提高患者满意度,因此值得临床普及推广。

Objective: To analyze the value of submental island flap in the treatment of oral and maxillofacial tumors and to observe the clinical effect.Methods: 36 patients with oral and maxillofacial tumors were selected from June 2013 to June 2016, depending on whether the patient had contraindications for radiotherapy and whether he would voluntarily receive radiotherapy, the selected patients were divided into control group (18 cases) and study group (18 cases), they all received treatment of submental island flap and enlarged resection of oral and maxillofacial tumor, lymph node dissection, on top of which, the study group performed tumor radiotherapy. The survival rate, recurrence rate, survival rate, postoperative complications and patient satisfaction were compared and analyzed.Results: The survival rate of the flap in the study group (94.44%) was not statistically significant (P>0.05). At the same time, the recurrence rate in the study group was lower than that in the control group; there was no significant difference in 1-,2-,3-year survival rate, the total satisfaction of the study group was higher than that of the control group. Analysis was statistically different (P<0.05). There was no significant difference in the probability of postoperative complications (P>0.05).Conclusion: Patients with oral and maxillofacial tumors treated with submental island flap and postoperative radiotherapy can achieve good results and will not have a serious impact on the survival of the flap. It can ensure the survival rate of the flap and control the recurrence rate of clinical tumors, as well as mprove patient satisfaction, so it is worth popularizing.
