实时三维超声心动图对下壁急性ST段抬高型心肌梗死合并右心室梗死的诊断价值研究

摘要:

目的: 探讨实时三维超声心动图(RT3DE)对下壁急性ST段抬高型心肌梗死(STEMI)合并右心室梗死(RVMI)的诊断价值。方法: 选择巴中市中医院下壁急性STEMI患者53例,按照是否合并RVMI分为RVMI组和非RVMI组,比较两组超声学参数。受试者工作特征(ROC)曲线评价右心室射血分数(RVEF)对RVMI的诊断价值。结果: RVEF诊断RVMI的曲线下面积为0.898,以RVEF=45.5%为截断值,诊断RVMI的敏感性和特异性分别为87.9%和85.0%。结论: 采用RT3DE检测RVEF诊断下壁急性STEMI合并RVMI具有较高的诊断价值。

Objective: To investigate the diagnostic value of real time three-dimensional echocardiography(RT3DE)in inferior acute ST-segment elevation myocardial infarction(STEMI) with right ventricular myocardial infarction(RVMI). Methods: A total of 53 patients with inferior acute STEMI who were hospitalized in Bazhong municipal hospital of traditional Chinese medicine were included in our study. According to patients with or without RVMI, we divided into RVMI group and non-RVMI group. Echocardiographic parameters were compared between the two groups. Diagnostic accuracy of right ventricular ejection fraction(RVEF) in detecting inferior STEMI with RVMI was evaluated by receiver operating characteristic(ROC) curve and calculated the area under curve(AUC). Results: A RVEF in diagnosing RVMI have high sensitivity and specificity with a cut-off value of ≤ 45.5%(AUC 0.898, sensitivity 87.9%, specificity 85.0%).Conclusion: This study shows that RVEF accessed by RT3DE provides a high diagnostic value for identifying RVMI in patients with inferior acute STEMI.
