非药物睡眠干预对重症监护成人患者谵妄和睡眠质量影响的系统评价

摘要：

目的：评价非药物睡眠干预措施对重症监护(ICU)成人患者谵妄、睡眠质量的影响。方法:计算机检索中国知网、万方、中国生物医学文献服务系统、维普、PubMed、Cochrane Library、CINAHL、Embase、Web of Science数据库中关于非药物睡眠干预对ICU成人患者谵妄和睡眠质量影响的文献，检索时间从建库至2022年7月31日。采用RevMan 5.3软件进行Meta分析。结果：共纳入13篇文献，总样本量1 866例，文献质量均为B级。眼罩耳塞可以减少ICU成人患者谵妄发生(RR=0.56，95%CI 0.33~0.95，P=0.03)。大部分研究结果表明非药物睡眠干预措施可以改善ICU成人患者的主观睡眠质量。结论：联合运用眼罩耳塞可能降低ICU成人患者谵妄的发生风险，但未来需进行高质量研究加强该证据。

Objective: To evaluate the effects of non-pharmacological sleep interventions on delirium and sleep quality in patients in intensive care units(ICU). Methods: We searched the databases of CNKI, Wanfang Data, SinoMed, VIP, PubMed, Cochrane Library, CINAHL, Embase and Web of Science from inception to July 31,2022 for randomized controlled trials on the effects of non-pharmacological interventions on delirium and sleep quality in adults patients in ICU. Meta-analysis was performed using RevMan 5.3 software. Results: A total of 13 articles were included, with a total sample size of 1 866 cases, all of which were B-grade. Eye patch and earplugs can reduce the incidence of delirium in ICU patients(RR=0.56, 95%CI 0.33-0.95, P=0.03). Most studies have shown that non-pharmacological sleep interventions can improve the subjective sleep quality in ICU adults patients. Conclusion: The combination of eye patch and earplugs may reduce the risk of delirium of ICU adults patients, but high-quality researches are needed to strengthen this evidence in the future.
