膀胱癌患者血清miR-125b和miR-1276的表达及其临床意义

摘要:

目的: 探讨膀胱癌(BC)患者血清中miR-125b、miR-1276的表达水平及其临床意义。方法: 以2017年5月至2019年5月收治的114例BC患者为研究对象(BC组),同时选取同期在开滦总医院林西医院进行健康体检者108例为对照组。比较BC组与对照组及不同临床特征BC患者血清miR-125b、miR-1276表达水平。分析BC患者血清miR-125b与miR-1276之间的关系。分析血清miR-125b、miR-1276对BC发生的预测价值及其与预后生存情况的关系;探究BC患者生存状况的影响因素。结果: BC组患者血清miR-125b、miR-1276水平显著低于对照组(P<0.05)。不同年龄、性别及肿瘤大小BC患者miR-125b、miR-1276水平无显著性差异(P>0.05),不同TNM分期、淋巴结转移、病理分级、浸润深度患者血清miR-125b、miR-1276水平有显著性差异(P<0.05)。Pearson相关性分析表明,BC患者miR-125b与miR-1276呈正相关(r=0.683,P<0.001)。血清miR-125b、miR-1276水平诊断BC的曲线下面积(AUC)分别为0.909、0.911,其灵敏度分别为90.4%、85.1%,特异度分别为74.7%、81.4%;两者联合诊断BC的AUC为0.963,其灵敏度、特异度分别为94.7%、83.6%。Logistic回归分析得出,血清miR-125b、miR-1276是影响BC患者生存状况的独立危险因素(P<0.01)。血清miR-125b及miR-1276高表达患者生存率均显著高于低表达组(χ21=17.384, χ22=19.859,P<0.01)。结论: BC患者血清miR-125b、miR-1276水平异常降低,且血清miR-125b、miR-1276水平较低者生存状况较差,因此血清miR-125b、miR-1276在BC患者血清中的表达水平可判定患者生存状况。

Objective: To investigate the expression of miR-125b and miR-1276 in serum of patients with bladder cancer(BC) and their clinical significance. Methods: A total of 114 patients with BC admitted from May 2017 to May 2019 were included into study group(BC group), meanwhile 108 patients who underwent physical examination in Linxi Hospital of Kailuan General Hospital were regarded as control group. The expression levels of miR-125b and miR-1276 in serum of the BC group and the control group and BC patients with different clinical characteristics were compared. The relationship between miR-125b and miR-1276 in serum of BC patients was analyzed to reveal the predictive value of serum miR-125b and miR-1276 for BC occurrence and their relationship with prognosis and survival. The factors influencing the survival status of BC patients were also explored. Results: The levels of serum miR-125b and miR-1276 in BC group were obviously lower than those in the control group(P<0.05).There was no obvious difference in the levels of miR-125b and miR-1276 in BC patients with different age, gender and tumor size(P>0.05), there were obvious differences in serum miR-125b and miR-1276 levels in patients with different TNM stages, lymph node metastasis, pathological grade and depth of invasion(P<0.05). Pearson correlation analysis showed that there was a positive correlation between miR-125b and miR-1276 in BC patients(r=0.683, P<0.001). The area under the curve(AUC) of serum miR-125b and miR-1276 levels for diagnosing BC was 0.909 and 0.911, respectively, sensitivity was 90.4% and 85.1% respectively, and specificity was 74.7% and 81.4% respectively. The AUC of combined diagnosis of BC was 0.963, and its sensitivity and specificity were 94.7% and 83.6% respectively. Logistic regression analysis showed that serum miR-125b and miR-1276 were independent risk factors affecting the survival status of BC patients(P<0.05). The survival rate of high expression of miR-125b and miR-1276 in serum were obviously higher than those of low expression(χ21=17.384, χ22=19.859,P<0.01). Conclusion: The serum levels of miR-125b and miR-1276 in patients with BC are abnormally low, in addition, those with low serum miR-125b and miR-1276 levels have poor survival conditions, therefore, the expression levels of miR-125b and miR-1276 in the serum of BC patients can determine the survival status of patients.
