Tergumed训练系统对慢性腰痛疗效的Meta分析

摘要:

目的:在脊柱生物力学测试与颈腰肌群个体化训练及慢性腰痛的治疗中,Tergumed训练系统十余年来得到逐步应用。本研究系统评价慢性腰痛治疗中Tergumed系统的临床效果。方法:计算机检索The Cochrane Library、Web of Science、EMBASE、PubMed、ClinicalTrails、CBM、CNKI、CQVIP、Wanfangdata、Chinese Clinical Trial Registry数据库及临床试验注册中心(建库至2023年2月),对Tergumed系统治疗慢性腰痛的随机对照研究进行检索。对文献进行方法学的质量评价,通过RevMan和STATA软件进行Meta分析。结果:Meta分析收录的6篇RCT试验,共纳入360人。Tergumed系统治疗慢性腰痛,干预组较对照组,疼痛视觉模拟评分(VAS)减轻不明显,效果不显著[MD=-0.39,95%CI(-0.89,-0.11),P>0.05];但腰背肌前屈、后伸等长最大肌力(IMS)改善明显,效果显著[前屈MD=60.51,95%CI(35.7,85.32),P<0.05;后伸MD=224.40,95%CI(175.66,273.14),P<0.05]。结论:现有研究显示,Tergumed系统显著增强了慢性腰痛患者腰背肌前屈和后伸IMS。

Objective: Over the past ten years, the Tergumed training system has been gradually applied in spinal biomechanical test, individualized training of cervical and psoas muscles and treatment of chronic low back pain. Meta-analysis was used to evaluate the efficacy of Tergumed system in the treatment of chronic low back pain. Methods: The Cochrane Library, Web of Science, EMBASE, PubMed, ClinicalTrails, CBM, CNKI, CQVIP,Wanfang Data,Chinese Clinical Trial Registry, databases and clinical trial registries were searched by computer(from database establishment to February 2023) to collect randomized controlled studies on Tergumed system in the treatment of chronic low back pain.The methodological quality of literature was evaluated and Meta-analysis was conducted by RevMan and STATA software. Results: All the finally enrolled 6 articles used a design of RCT. Meta-analysis showed that, compared with control group, VAS score did not significantly decrease in the intervention group after treatment [MD=-0.39,95%CI(-0.89,-0.11),P>0.05]; But the isometric strength(IMS) of trunk muscle was improved significantly[Strength of flexor muscles MD=60.51,95%CI(35.7,85.32),P<0.05; Strength of extensor muscles MD=224.40,95%CI(175.66,273.14),P<0.05]. Conclusion: Current studies have shown that the Tergumed system can significantly enhance IMS of trunk muscle flexion and extension in patients with chronic low back pain.
