TyG和TyG-BMI与高血压患病的相关性分析

摘要:

目的:探讨甘油三酯葡萄糖(TyG)指数以及甘油三酯葡萄糖体质指数(TyG-BMI)与高血压的相关性。方法:纳入2019年1月至2020年12月在南京市医疗机构中进行健康体检的35岁以上人群为研究对象,收集其一般人口学特征、疾病史、生活习惯等情况,并测量身高、体重、血压,记录其生化指标、血糖等指标,根据高血压诊断标准将体检人群分为高血压组与血压正常组,比较两组的一般情况的差异。采用Pearson相关分析以及多因素Logistic回归分析TyG、TyG-BMI与高血压患病的关系。结果:共纳入917例体检对象,其中男524例,女393例,平均年龄(57.13±14.07)岁。Pearson相关分析结果显示,随着TyG和TyG-BMI水平不断增加,收缩压(SBP)与舒张压(DBP)水平也不断增加。高血压患病率随TyG和TyG-BMI水平增加而增加,差异有统计学意义(趋势χ2分别为34.510、108.511,P<0.001)。进一步进行Logistic回归分析,结果显示TyG和TyG-BMI均是高血压的独立危险因素,高血压患病风险与TyG和TyG-BMI浓度均呈正相关(OR=1.839,95%CI:1.254~2.695;OR=1.021,95%CI:1.014~1.028)。亚组分析结果显示,TyG-BMI第三分位数组高血压患病风险是第一分位数组的2.366倍(95%CI:1.189~4.708),第四分位数组高血压患病风险是第一分位数组的5.780倍(95%CI:2.855~11.699)。ROC曲线分析结果显示,TyG预测高血压的截断值为8.99,曲线下面积为0.630,灵敏度为0.378,特异度为0.850;TyG-BMI预测高血压的截断值为216.56,曲线下面积为0.710,灵敏度为0.592,特异度为0.733。结论:TyG、TyG-BMI与血压水平呈线性正相关,且是高血压的独立危险因素,TyG-BMI预测高血压发生风险的价值高于TyG。

Objective: To investigate the correlation between triglyceride glucose(TyG) index and triglyceride glucose body mass index(TyG-BMI) withhypertension.Methods: People over 35 years old who underwent physical examination in medical institutions in Nanjing were included in the study from January 2019 to 2022 December. According to the diagnostic standard of hypertension, the subjects were divided into hypertension group and hypertension normal group.General demographic characteristics, disease history, life habits and other information were collected, height, weight, blood pressure and biochemical indicators, blood glucose and other indicators were measured, and the differences in general conditions were compared with two groups. Pearson correlation analysis and Logistic regression were used to analyze the correlation of TyG, TyG-BMI with prevalence of hypertension,respectively.Results: A total of 917 people were included in this study, including 524 male and 393 female, with the average age of 57.13±14.07.Pearson correlation analysis showed that with the increasing of TyG and TyG-BMI levels, the levels of systolic blood pressure(SBP) and diastolic blood pressure(DBP) also increased.And the prevalence of hypertension also increased with the increase of TyG and TyG-BMI groups according to quartile, and the trend chi-square difference was statistically significant(χ2=34.510,108.511,P<0.001). Further Logistic regression analysis showed that TyG and TyG-BMI were both independent risk factors for hypertension, and the risk of developing hypertension was positively associated with both TyG and TyG-BMI concentrations(OR=1.839,95%CI: 1.254-2.695; OR=1.021, 95%CI: 1.014-1.028). Grouped by quartile, the risk of hypertension in the TyG-BMI third quartile group was 2.366 times than that in the first quartile group(95%CI:1.189-4.708), and the risk of hypertension in the fourth quartile group of TyG-BMI was 5.780 times than that in the first quartile group(95%CI:2.855-11.699). ROC curve analysis results showed that the truncation value of TyG in predicting hypertension was 8.99, the area under the curve was 0.630, the sensitivity was 0.378, and the specificity was 0.850. The cutoff value of TyG-BMI for predicting hypertension was 216.56, the area under the curve was 0.710, the sensitivity was 0.592, and the specificity was 0.733.Conclusion: TyG and TyG-BMI are linearly positively correlated with blood pressure, and are independent risk factors for hypertension. TyG-BMI is more valuable than TyG in predicting the risk of hypertension.
