补充益生菌对成人代谢综合征患者糖代谢影响的系统评价

摘要:

目的:探究益生菌补充剂对成人代谢综合征患者糖代谢的影响,为代谢综合征患者异常糖代谢的治疗提供新的方向。方法:检索Web of Science、PubMed、Embase、Cochrane、中国知网(CNKI)、万方数据库、中国生物医学文献数据库(CBM)等数据库,收集截至2023年7月的补充益生菌对成人代谢综合征糖代谢影响的随机对照试验。采用RevMan 5.4.0软件进行统计学分析。结果:共纳入了12项随机对照试验,包括632例受试者。分析结果显示,与对照组相比,补充益生菌的治疗可降低代谢综合征患者的空腹血糖(SMD=-0.17,95%CI:-0.32~-0.01,P=0.04)和空腹血浆胰岛素(SMD=-0.17,95%CI:-0.34~0.00,P=0.05),差异具有统计学意义。然而,胰岛素抵抗指数的改变在两组中比较差异未见统计学意义(SMD=0.07,95%CI:-0.18~0.31,P=0.61)。结论:补充益生菌可以改善成人代谢综合征患者的糖代谢情况,特别是对空腹血糖和空腹血浆胰岛素具有积极的效果。然而,对于胰岛素抵抗指数的积极作用尚未得出明确结论。补充益生菌可能成为治疗成人代谢综合征患者异常糖代谢的一种新方向。

Objective: To explore the effects of probiotic supplementation on glucose metabolism in adult patients with metabolic syndrome, provide new directions for the treatment of abnormal glucose metabolism in patients with metabolic syndrome.Methods: The Web of Science, PubMed, Embase, Cochrane, CNKI, Wanfang database, and China Biology Medicine(CBM) databases were searched to collect randomized controlled trials on the effects of probiotic supplementation on metabolic syndrome in adults up until July 2023. Statistical analysis was performed using RevMan 5.4.0 software. Results: A total of 12 randomized controlled trials involving 632 participants were included. The analysis results showed that compared to the control group, probiotic supplementation reduced fasting blood glucose(SMD=-0.17, 95% CI:-0.32- -0.01, P=0.04) and fasting plasma insulin(SMD=-0.17, 95%CI:-0.34-0.00, P=0.05) in patients with metabolic syndrome, with statistically significant differences. However, no statistically significant difference was observed in the difference in terms of change in insulin resistance index between the two groups(SMD=0.07, 95%CI:-0.18-0.31, P=0.61). Conclusion: Probiotic supplementation can improve glucose metabolism in adult patients with metabolic syndrome, particularly in terms of fasting blood glucose and fasting plasma insulin. However, the positive effects on insulin resistance index have not been conclusively determined. Probiotic supplementation may serve as a new direction for the treatment of abnormal glucose metabolism in adults patients with metabolic syndrome.
