血清长链非编码RNA DANCR预测急性心肌梗死患者主要不良心血管事件的价值

摘要:

目的:探讨血清长链非编码RNA(lncRNA)DANCR预测急性心肌梗死(AMI)患者主要不良心血管事件(MACE)的价值。方法:选择2022年1月至2023年12月在本院就诊的71例接受经皮冠状动脉介入治疗(PPCI)的AMI患者为研究对象。所有患者于PPCI术后次日清晨检测血清lncRNA DANCR水平。随访3个月后,根据是否发生MACE分为MACE组和无MACE组,比较两组患者血清lncRNA DANCR水平的差异,并评估其预测MACE的价值。结果:71例患者中33例发生MACE。MACE组血清lncRNA DANCR水平显著低于无MACE组(P<0.01)。二元Logistic回归分析发现,多支病变数量、hs-cTnT峰值及lncRNA DANCR水平均为AMI患者发生MACE的独立危险因素(P<0.05)。受试者工作特征(ROC)曲线显示,lncRNA DANCR预测AMI患者MACE的曲线下面积(AUC)为0.778(95%CI 0.671~0.885)。Pearson相关分析表明,血清lncRNA DANCR水平与左室射血分数(LVEF)和左室短轴收缩率(LVFs)呈正相关(r=0.796、0.743,均P<0.01),而与左室舒张末内径(LVEDD)及血浆脑利钠肽(BNP)水平呈负相关(r=-0.789、-0.785,均P<0.01)。此外,单因素线性回归分析同样显示,lncRNA DANCR水平与LVEF及LVFs呈正相关(P<0.01),而与LVEDD及BNP呈负相关(P<0.01)。结论:AMI患者PPCI术后血清lncRNA DANCR水平下降是预测MACE发生的独立危险因素,同时预示较差的心功能。

Objective: To explore the value of serum long non-coding RNA(lncRNA) DANCR in predicting major adverse cardiovascular events(MACE) in patients with acute myocardial infarction(AMI). Methods: A total of 71 AMI patients who underwent percutaneous coronary intervention(PPCI) at our hospital from January 2022 to December 2023 were included in the study. Serum lncRNA DANCR levels were measured on the morning following the PPCI procedure. Patients were followed up for 3 months and divided into MACE and non-MACE groups based on the occurrence of MACE. The difference in serum lncRNA DANCR levels between the two groups was compared, and the value of predicting MACE occurrence was assessed. Results: Among the 71 subjects, 33 experienced MACE events, and the serum lncRNA DANCR levels in the MACE group were significantly lower than those in the non-MACE group(P<0.01). Binary Logistic regression analysis revealed that the number of multi-vessel lesions, peak hs-cTnT levels, and DANCR levels were independent risk factors for MACE in AMI patients(P<0.05). The receiver operating characteristic(ROC) curve showed that the area under the curve(AUC) of DANCR for predicting MACE in AMI patients was 0.778(95% CI 0.671-0.885). Pearson correlation analysis demonstrated that serum lncRNA DANCR levels were significantly positively correlated with left ventricular ejection fraction(LVEF) and left ventricular fractional shortening(LVFs)(r values of 0.796 and 0.743, P<0.01), and negatively correlated with left ventricular end-diastolic diameter(LVEDD) and plasma BNP levels(r values of -0.789 and -0.785, P<0.01). Furthermore, univariate linear regression analysis also showed DANCR levels had significant positive correlations with LVEF and LVFs(P<0.01), and significant negative correlations with LVEDD and BNP(P<0.01). Conclusion: A decrease in serum lncRNA DANCR levels after PPCI in AMI patients is an independent risk factor for predicting the occurrence of MACE and indicates poorer heart function.
